Fort Healthcare is a NDIS Registered Provider

We’re provding both NDIS Assessments and Interventions services, including:

  • Theraputic Supports (CB – Improved Health and Wellbeing, CB – Improved Daily Living)
  • Specialist Behavioural Intervention Support (CB – Improved Relationships)
  • Behaviour Management Plan including Training in Behaviour Manageement Strategies (CB – Improved Relationships)
  • Early Childhood Intervention 

We’re providing Allied Health Supports that help you go about your daily life, and are related to your disability-related impairments that impact your functional capacity.  

We’re providing both in-clinic NDIS services and Mobile & Home visit services across ALL the regions of Sydney.

This includes maintenance supports, given or supervised by qualified healthcare staff. These are ongoing supports that help you keep your body’s functions and abilities. This could include long term therapy or support, to improve your abilities over time or stop things from getting worse.

Assessments

Our Clinical Psychologists and Neuropsychologists provide assessments including Autism Spectrum Disorder (ASD) and Attention-deficit/hyperactivity disorder (ADHD) in both children and adults.

We’re providing Assessments for both NDIS Application and Review purposes, including:

  • Occupational Assessments
  • Speech Pathology Assessments

What is the National Disability Insurance Scheme?

The National Disability Insurance Scheme is usually called the NDIS. It is a new way of providing support to people with disability in Australia.

The NDIS is based on the idea that everyone with disability is different and that they need different types of support. It gives people with disability more choice and flexibility over the supports they use.

Who can use the NDIS?

The word ‘disability’ can mean many different things. To access services through the NDIS, you will have to meet some requirements. To be eligible for NDIS services, you will need to:

  • be aged between 7 and 65 years
  • be younger than 7 years with a disability or a developmental delay — this group and their families will be supported through the early childhood approach
  • live in Australia and have Australian residency
  • usually need support due to a permanent and significant disability
  • use special equipment because of a permanent and significant disability
  • need some supports now to reduce your future needs
