We provide comprehensive occupational therapy assessments for both NDIS and Home Care Package Participants.
We provide both in clinic and mobile services.

NDIS Occupational Therapy Assessments
At Fort Healthcare, our experienced Occupational therapists provide assessments and reports, including but not limited to:
- Functional capacity assessment (FCA): are often requested to determine the supports a NDIS participant may require and to make recommendations for an NDIS Plan Review to identify priorities and goals
- Assistive Technology (AT) Assessment
- Equipment prescription assessment
- Simple Equipment Prescription Assessment
- Home Modifications Assessment
- Supported Living Assessments
- Skills Development Training Program
- Risk & Manual Handing Assessments: to determine the level of support a participant may require
- Sensory Assessment: participants with ASD often require a Sensory assessment to support NDIS funding application
- Support Letters

Home Care Package (HCP) Occupational Therapy Assessments
At Fort Healthcare, our experienced Occupational therapists provide assessments and reports, including but not limited to:
- Self-care assessment including: self care, domestic tasks and instrumental tasks
- Enviornment assessments
- Home Modification assessments
- Assistive Technology or adaptive equipment assessments: such as specialised seating, mattresses, shower chairs, hoist and sling , scooter and so on
- Fall prevention assessments
- Pressure care
- Cognitive assessment
- Manual Handling training and education
- Support Letters
Our OTs can provide the HCP services at our clinics or at your home or anywhere you prefer.

Our Occupational Therapists' Key Roles
Assessment:
- Evaluate a person’s strengths, weaknesses, and limitations in performing everyday activities.
- Analyze the environment and identify any barriers to independence.
- Develop a personalized treatment plan based on the assessment findings.
Treatment:
- Provide interventions to improve a person’s ability to perform daily activities, such as:
- Self-care tasks (bathing, dressing, grooming)
- Home management (cooking, cleaning, laundry)
- Work or school activities
- Leisure activities
- Use various techniques and equipment, such as:
- Adaptive devices and assistive technology
- Sensory integration activities
- Exercise and movement programs
- Cognitive-behavioral therapy
Education:
- Educate patients and their families about their condition and how to manage it.
- Assist patients in setting realistic goals and developing coping mechanisms.
