Privacy Policy
We respect your privacy rights and take our privacy obligations seriously. We comply with the Australian Privacy Principles under the Privacy Act 1988 (cth) ,and other privacy laws which governs the way in which organisations hold, use and disclose personal information(including your sensitive information).​
Fort Healthcare collects information from you for the provision of integrated healthcare services to you. This includes collecting your personal information such as your name, your full medical history and any other information which is necessary to assist our allied health team in providing healthcare service that you need.​
Billing Policy
For Medicare bulk billing services, all patients must produce a current Medicare card and EPC form referred by your GP at the initial time of consultation. If you don’t produce a current Medicare card and EPC form, we cannot provide bulk billing services.
If your EPC form is not available (missing or stolen) or you have used up 5 times of Allied Health Services (if you uses the Medicare services at other clinics, we consider it is your responsibility to remember the times you have used the services), a full consultation fee can be paid at the time of consultation.
Cancellation Policy
We appreciate time, including yours and ours. This cancellation policy allow our clinics to help more people achieve their recover.Â
Please do not rely on SMS/call reminders – they are a complimentary service. We consider it is your responsibility alone to remember your appointment. Please feel free to ask for an appointment card.
From January to September, if patients cancel their scheduled appointments within 24 hours, Fort Healthcare reserves the right to charge the patient a full consultation fee.
From October to December, if patients cancel their scheduled appointments within 48 hours, Fort Healthcare reserves the right to charge the patient a full consultation fee.
Fort Healthcare reserves the right to request pre-payment on arrival or before an appointment.
Continued dismissal of our attendance policy may result in a client being refused at Fort Healthcare.
Exceptions may be made for immediate emergencies only, such as medical emergencies, car breaking down on the way. This does not include reasons such as last minute work, change of plans, forgetfulness and so on.
